What “no-gap” (preferred provider) actually means

“No-gap” (sometimes called “preferred provider”, “Access Gap”, “Members’ Choice”, or “Members First”) means that Dr Goutham has signed an agreement with the health fund to accept their schedule of fees in full. For your surgeon’s fee, that means $0 out-of-pocket — the fund pays the entire surgeon’s cost and Dr Goutham agrees not to bill you any extra.

Three things matter for true no-gap:

Hospital-level cover

You hold hospital-level cover with one of the participating funds — not just “extras” cover.

Agreement with surgeon

Dr Goutham has an active no-gap / preferred-provider agreement with your fund for the specific procedure.

Contracted facility

The procedure is performed at a participating private day-surgery facility (Springwood, Brisbane South Private, St Andrew’s, Mater Redland or North Lakes).

The surgeon’s fee is one component of the total cost. Even with a no-gap agreement on the surgeon’s fee, there may still be a small gap for: anaesthetist (separate contract, sometimes no-gap with your fund and sometimes not), pathology if biopsies or polyps are taken, and any excess attached to your hospital cover.

Each major Australian health fund has its own name for the same idea. Whichever fund you’re with, the practical effect is the same — no out-of-pocket cost for the surgeon’s fee.

Bupa — Bupa Members First / No Gap

Dr Goutham is a Bupa Members First / no-gap provider for colonoscopy and gastroscopy. Eligible Bupa members holding hospital cover that includes gastrointestinal endoscopy pay no out-of-pocket cost for the surgeon’s fee when the procedure is done at a Bupa-contracted facility. HCF — HCF More for Members

Medibank Private & ahm — Members’ Choice Advantage / GapCover

Hospital-level cover requirements

From April 2019, the Australian government standardised hospital cover into Basic / Bronze / Silver / Gold tiers (plus a “Plus” variant that adds extra services). Each tier defines which clinical categories are included. Colonoscopy and gastroscopy fall under “Gastrointestinal endoscopy”, which is typically included from Bronze Plus upwards.

If you have Basic or Basic Plus cover that excludes endoscopy, you have two options: (1) wait out an upgrade waiting period (typically 2 months for a higher tier, longer for pre-existing conditions), or (2) proceed as a self-funded patient at the published fixed-price package — see the cost page.

The facility matters too

No-gap arrangements only apply at contracted private day-surgery facilities. Dr Goutham’s operating network — St Andrew’s War Memorial Hospital, Mater Private Hospital Redland, Springwood Day Surgery, Brisbane South Private Hospital and North Lakes Day Hospital — comprises contracted facilities for all major funds. How to confirm with your fund before the procedure

The fund-side check is the patient’s responsibility. The most reliable way to be certain is a 5-minute phone call to your fund, with four specific questions:

  1. “Am I covered for gastrointestinal endoscopy on my current policy?” — this confirms your cover level includes the procedure.
  2. “Have I served the waiting period for this procedure?” — usually 2 months for new conditions, 12 months for pre-existing.
  3. “Is Dr Goutham Sivasuthan (provider number [TBD — Dr Goutham to confirm]) a no-gap provider with my fund for Medicare items 32222, 32223, or 32229?” — quote the item numbers for diagnostic and surveillance colonoscopy / gastroscopy / combined procedure.
  4. “Is [insert facility — e.g. Springwood Day Surgery] a contracted hospital for my cover, and what is my hospital excess?”

Frequently asked questions

Why are some surgeons “no-gap” with some funds and not others?

Each fund has its own schedule of fees, contract terms and administrative process. To be a no-gap provider with a particular fund, the surgeon agrees to accept that fund’s schedule in full and not charge any extra. What if my fund changes its agreement with Dr Goutham between consultation and procedure?

Provider agreements are reviewed periodically and changes are uncommon. Where they do happen, we notify affected patients in advance and reconfirm cover before the procedure. The fund-side confirmation you obtain (Step 3 of the four-question check above) is your strongest protection.

Is no-gap the same as bulk-billing?

No. Bulk-billing is a Medicare arrangement where the doctor accepts the Medicare rebate as full payment. No-gap is a private-health-insurance arrangement where the fund pays the surgeon’s fee in full. The two are different funding pathways but the practical effect for the patient — no out-of-pocket fee — can be the same. Dr Goutham’s no-gap arrangement is via private health insurance.

I have hospital cover but I’m within my waiting period — can I still get a no-gap colonoscopy?

Generally no — if you haven’t served the relevant waiting period, your fund won’t pay the hospital component and you’ll be liable for a much larger out-of-pocket. There are two exceptions: (1) emergency procedures, and (2) some funds waive waiting periods for symptomatic patients with red-flag indications — phone your fund.

Why does the anaesthetist sometimes still charge a small gap?

Anaesthetists at the day-surgery facilities are independent practitioners with their own fund contracts. Most are no-gap with most funds, but a small gap of $30–$150 sometimes applies. What about the Overseas Visitors Health Cover (OVHC) or international insurance?

The major OVHC providers (Bupa OVHC, Medibank OVHC, nib OSHC, Allianz Care Australia) generally treat scopes the same as their domestic policies. International insurance policies vary widely — phone the practice for an individual assessment.

Can I switch funds if mine doesn’t cover endoscopy?

Yes, but waiting periods for pre-existing conditions transfer with you and switching mid-investigation rarely speeds anything up.
